Medical Form Nightmare - this past Thursday

UGH!!! I arrived 15 minutes early for my visit to my nurse practioner. My appt was for 7:15. She saw me at 8:40. Yes, I was late for work which starts at 9. Okay, I'll live with that.

I gave my adoption medical paperwork to the woman working the front office, she in turn gave it to the office manager. The office manager called me into her office and explained she couldn't have the nurse practioner fill out the forms. How could the nurse practioner (NP) know if there is any history of hereditary disease or abnormality( physical disability) or mental health? How can she answer if I have any medical conditions that would limit the applicants' ability to parent an adopted child, as asked on my homestudy medical report form. How can she certify that I'm in good health with no communicative or mental disease/disability and am able to care for an adopted child, as asked by my international agency?

In my head, I was screaming, is she for real? Are we really having this discussion? And how could she ask the NP to sign the forms when under the signature line it says doctor/physician signature?

Taking a deep breath, I explained these are the same forms signed by the NP two years for my foster care agency. I got the impression she wanted to say to me that was then, this is now, but refrained. She told me that I should tell my agency to reword their signature line with health care professional or something of that language. Because as it stood now, this would be the same as if she had a piece of paper that required the signature of a lawyer and she had her mother sign it. WHAT??!! I bit my tongue, but I was dying to ask if her mother was even in the legal field for that analogy or example to make any sense.

The office manager said when she received the test results, she would feel comfortable having the NP sign a letter saying that (sorry for the male readers) that my PAP was clear and that I didn't have any STDs or high blood pressure.

I nodded my head and I told her I would get back to her and I thanked her as I walked out of her office.

Hours later...

Once I calmed down, I called my insurance company to find a doctor. Snap! I found one that was close to my house and saw patients at 7AM. As I was making the appointment, the scheduler said, you do know the first meeting with the doctor is a consultation, then you can make an appointment and the second visit you can have a physical. CONSULTATION??!!!! What are we dating? I want a physical and for you to sign these papers that's it. UGH!

So now I'm putting in a call to speak directly to the NP.

I'm so glad you guys are hear to listen. Who know these forms were so complicated and so UN-PC?

Oh no! So sorry you had to go through all of that, what a mess!! All I did was send my medical form in-my doctor signed it and got it noterized for me and sent it back. I hope the process is smooth from now on for you!!!

All the Vietnamese require is a medical certificate, signed by a doctor, on his headed paper. The wording on my cert stated " That Brendan O'Donoghue was mentally and physically capable of adopting" I am sure that you will find some doctor that will sign it for you.

Oh, do I feel your pain!

I had the same trouble trying to get my med forms notarized by my MD for my HS. The office assistant informed me that since they dont have a notary in the office, that maybe if I tried the banks in the area, that one of them probably has heard of Dr____, and maybe they will just notarize it for me.

What?????

So I said "you mean to tell me to go around town and ask the people in the banks if the know of Dr____???"

SHE SAID YES!!!!

Now I have to go back with new forms for the Dossier this Friday and its making me all nuts already!

Hi Chriso,

I think you are getting a bit mixed up.

Doctors cannot notorise documents.

What we all did was to get all of our documents, financials, medicals etc all together, then bring all the documents to a notary when you are finished.

He notorises the documents that they are true original or copy documents. He does not have to know the doctor or his signature.

If you are notorising single documents at a time it will work out very expensive.

Call a Notary today and explain the situation that you will need a lot of documents notarised and what should you do.

Remember when you are adopting in Vietnam all documents should be 6 months in date. Leave everything as late as you can or else you are going to have to go back and get all the documents re-issued and notarised again.

I would open a new thread on different lists and see what opinions you get.

lol No doctors don't notarize things here either..but I think she was talking about having the doctor's signiture notarized lol

Here in Texas most notariers will NOT notarize something unless they SEE the person sign it in front of them. I do have a family friend who is a notary and will take care of a few things for me IF it is a person she knows. BUT normally here the signing person MUST sign infront of the Notary.

Sorry ya'll are having such problems!! I need to start looking at the papers I'm going to need for my homestudy, I want to say that It's going to be easier for me as I'm currenting living in my home state and I'm single, so no marriage license or divorce decrees etc. just a Birth Cert. lol

Hope you get it taken care of SOON!!!!

Thanks everyone for your replies. I thought it would be so easy for the NP to sign, but not according to the office manager. I'm going to call the NP directly and see if she'll overstep the wacky Office Manager. Maybe I should fax over a copy of the last medical form they signed for me so I could become a fost/adopt parent.

Brendanod, I read out loud what the letter had to say for VN. The best she could do was a letter saying that my PAP was clear and that I didn't have chylamedia(sp?) or gonnerhea (sp?). She couldn't say all STDs. I just nodded and told her if I needed that particular letter on letterhead.

Yeah, Shana........

thats what I meant. And the thing that irritates me the most is that there are three banks like right across the street from the Drs office, and he could get those forms notarized for me so quick! Instead I get a song and a dance about how busy they are and how they dont have time for that.

Ugh. If I have to this time I'll just have to pay a traveling notary to go to the office with me I guess.

At least I know that this will be the last time I'll have to deal with this.

I'm actually pretty surprised. Most of the time the Office manager at the Dr. office IS a notary. Doctors have to sign and have things notorized ALL the time. Just about anything you do for Retirement, Insurance, Disablity etc has to be notorized. lol

If ya'll are having real issues with the office managers I would suggest taking your letter right in to the doctor and saying "this is what I need EXACTLY" I know my doctor will be HAPPY to have something he can just hand his office assistant and say "type this up for my signature please" It hard then they have to make it up on their own and it actually takes some of their time lol

As for your NP, Ask her if her supervising Dr. will sign it for you. When I was working for a Dr. he did that all the time for his NPs for insurance forms etc. I'm sure it would take them a whole 5 minutes to look thorugh your chart and sign off on it!
