Need Help with Medical Form for UA
We just had our physicals completed for our adoption. I have an appointment with my Dr. to fill out the form required by Ukraine for all prospective adoptive parents. Could someone please give me some guidance on what numbers the Dr. should use for the Wasserman and HIV test results. Since both tests were done at the same time I have only one "specimen" number on my report for each of us. I'd really, really appreciate anyone who's already jumped through this hoop to let me know what they did. Thanks!!! Lisa S.

Dear Lisa, The number that they are looking for is the Patient Number the Lab uses to identify you. This number should be on the lab report. It could be your SS# or a lab specific account or specimen number. It sounds like your lab uses specimen numbers to identify you. Still, look over your report. If there are any other identifying numbers on it besides the specimen number, ask your doctor, "Which number would the lab use to look up my lab results." If they use one number to pull up the results of both tests, then the Doctor would fill-in that one number on both the HIV line and the Wasserman reaction line. CLHayes |
