Need "General Judgement of Adoption" form for Oregon

The only form I haven't come up with examples or samples for. Anyone with any helpful information, even out of state forms, would be greatly appreciated! R

Smile adoption forms

We did our adoption 'pro se' in NJ, and found all the forms in the law library of a college here, but they were very difficult to find, and if it weren't for the help of some very kind hearted young law students, i never would have found them in a million years...also, i was advised that a paralegal who specializes in family law can get you these forms, for a much smaller fee than a lawyer, and you can still represent yourself...if your adoption is uncontested, there really is no need for a lawyer anyway. Hi, My husband and I are just starting our step-parent adoption so he can adopt my son Shawn. We have filled out all forms available to us, except haven't found the General Judgement of Adoption. I did find an example Petition for North Carolina and plan on going to the law library to see if it fits ORegon's criteria, however, everytime I speak to anyone down there on the phone they say I have to hire and attorney..which is untrue. I would be very interested in seeing anything you have to offer, it would be immensely appreciated!
