Forms Needed?

My fiance is looking to adopt my twin boys. Their dad is consenting to the adoption and i am looking for what form I need to start the adoption process. I live in IL.

hopefully this will help you

I don't know where to find the form's for your state, but I went to my state homepage & clicked on site-map, then downloadable form's. After saearching through the form's I found what I needed for step-parent adoption. To find your state homepage call the state or type in "your state" homepage, such as (Illinois state homepage)


The form's I downloaded included-

* Schedule of filiing fee's

*Petition to the court for step-parent adoption

*Affidavit of waiver of parental right's, relinquishment of child, & consent for step-parent adoption (all in 1 form) this form is to be filled out by the absent parent giving consent

*Consent of the "CUSTODIAL" parent allowing the child to be adopted by the step parent (your spouse/signifigant other)

*Decree for adoption for the judge to sign allowing the adoption, which included the termination of absent parent's right's, & allowing the name change.

Also- My mom has a friend in Arizona who went to the county attorney for the form's. The county attorney provided these form's to them for free. Final cost's came to around $300.

Don't call & ask for the form's. Go down there & insist on speaking with the county attorney or one of the other public defender's. We as tax payers pay their wages & they are here to help us, not to blow us off.

These are legal form's & each state/county differ's from the other. The county does have or know's where to obtain these form's at no cost. Don't give up
