how do we get an adoption petition???

my husband wants to adopt my daughter, he has been with us for over five years, and my ex husband said if thats whatwould make her happy then he will agree to it also. now we need to know how to get the petition for adoption. we went to the courthouse and they said they didnt have any, and that they have never seen someone do it without a lawyer, so i thought maybe i could get an answer here...........any help would be great(we live in utah)

The Petition to do Step Parent Adoption Yourself is Online. All that you would then need to do is file it with the Court.

i have looked online for that but all we ever find are stories about other adoptions. do you happen to know a link you could share with me? thanks!

Wow, Utah does not have anything online that I could find either. You may need a lawyer just to draw up the papers. I would call around to county courthouses to get advice from them on where to get the forms.
