Question Change Birthcertificate/Social Security Card

I have a few questions and am looking for answers. My husband just adopted my daughter last month. How do I go about getting her birthcertificate changed and her social security card updated to reflect her new name? Also, what documentation do I need to get her name changed on her medical records as well? I don't know if this is something that my lawyer is supposed to do (I don't have a whole lot of faith in him) or something that I need to do. Thanks in advance for your help.

For the social security card, go to the SS office(or print the forms on line) and fill out forms. They'll need documents with her new and old last name. They will take the adoption decree as one form. It gives a list on line of what they'll accept. It's faily simply.
(don't know the web address, just type it in the search engine).

As fot the other records, the adoption certificate should be enough.

We were told our lawyer does NOT do these steps so the changes will be up to us to make.
lucyjoy is correct on the SS#'s it is very simple. As for the birth certificates I think you need to apply for an amended certificate to be sent to you....just like we did at birth and we had to mail in a check.
I am not sure about the medical records...I do know that the CDC tracks immunization records which come from the doctor. Also medical insurance would have a legal name.... I personally, think that to avoid any confusion I would get the Birth Cert, change SS#, change medical info and the school records. It would be terrible if things got all jumbled up later.....like when she applies for student aid or loans and then there is a delay and well... Do all the things you did when you got married to make sure you were who you are...plus the birth cert.....that is if you took your husbands name

My lawyer did do the birth certificate and I did get those first before amending the SS card.

birth Certificate

Call your court administrator. She/he can give you the forms you need to get a new birth certificate. You can have them send a certified copy of the adoption decree. My court administrators office helped me fill the whole form out. There was a filing fee form me but I am in Minnesota and I am not sure where you are from. It ended up being $33 for me. Then when I got them I just went to the local Social Security office and they ask you some questions to verify who you are. You show them a ceritfied copy of the adoption and the birth ceritficate and they will change it and you will receive it in a couple of weeks. Good Luck!!
