Hospital Birth Records

Is it possible to send a non-notarized letter to the hospital requesting birth records and still get results? I know what my my birth name was but can't get my letter notarized because naturally my real name is different from my birth name. Any ideas how to get around this problem?

I to have alot of information on my birth-mom. I also no my name in the hospital at birth was Baby Girl Levy. I was born in Philadelphia. I contacted the hospital, and they told me to just send my moms name and my birthday and my SS# and also a proof of ID just for signature purposes. They did not say I had to get the letter noterized. Hospital requirements for requests for records vary from hospital to hospital so it is best to check with the hospital first like "blandp" did.

Here are at least 3 web sites where oyu can do searches for people. The people-data will search in last known location, and they charge about $10 for their info. Whowhere is a free search and will give names and addresses of people. www.people-data.com www.whowhere.com www.whitepages.com Hope these might help a bit. I read an email of a woman who found her adopted daughter by getting a list of names from Whowhere and sending them all a form letter - - one responded and it was her daughter!
