I to have alot of information on my birth-mom. I also no my name in the hospital at birth was Baby Girl Levy. I was born in Philadelphia. I contacted the hospital, and they told me to just send my moms name and my birthday and my SS# and also a proof of ID just for signature purposes. They did not say I had to get the letter noterized. Any questions please contact me at blandp@hotmail.com

Hospital requirements for requests for records vary from hospital to hospital so it is best to check with the hospital first like "blandp" did.

Smile To Norayne Hardy

Norayne:

Here are at least 3 web sites where oyu can do searches for people. The people-data will search in last known location, and they charge about $10 for their info.
Whowhere is a free search and will give names and addresses of people.

www.people-data.com
www.whowhere.com
www.whitepages.com

Hope these might help a bit. I read an email of a woman who found her adopted daughter by getting a list of names from Whowhere and sending them all a form letter - - one responded and it was her daughter!
Very much luck to you on your search. Kats45
