Arizona Searching Resources
Quote:
Welcome to the Search and Reunion Message Board! NOTE: The State of Arizona has a Confidential Intermediary Search Program that will locate and contact birth relatives! Using a Confidential Intermediary: The following parties may use the services of a confidential intermediary: Birth siblings age 21 or older; adoptive parents or legal guardians of adopted adults age 18 or older; adopted adults age 18 or older; the immediate, adult relatives (age 21 or older) of a deceased adopted adult; birth parents; or birth grandparents if birth parents are deceased. A confidential intermediary must obtain written consent from both parties before releasing information. Adoptive parents may file an affidavit that prohibits the confidential intermediary from contacting the adopted adult unless the adoptive parents remove the affidavit or the adopted adult files an affidavit stating that he or she knows he or she is adopted and wishes to make contact with the birth parents. Birth parents can file an affidavit prohibiting contact with birth siblings. It is important to note that the Confidential Intermediary program can only take cases finalized in Arizona. If an adoptee was born in Arizona but adopted in a different state the Arizona C.I. program is not available to those persons. Contact the adoption agency that handled the adoption or the office below: Arizona Confidential Intermediary Program Arizona Supreme Court 1501 W. Washington Phoenix AZ 85007 (602) 364-0357 TDD (602) 542-9545 This website link has the search application and a list of intermediaries: http://www.supreme.state.az.us/cip OTHER ARIZONA RESOURCES: For questions about access to adoption related information and records: Arizona Department of Economic Security Children, Youth & Families Division PO Box 6123, Site Code 940A Phoenix, AZ 85005 (602) 542-5499 Fax: (602) 542-3330 Obtaining Non-Identifying Information: Non-identifying information is available to adoptive parents or a child's guardian, adopted adults age 18 or older, an adopted adult's spouse, birth parents, birth siblings, and-if an adopted adult is deceased-his or her adult children (age 18 or older). Contact the adoption agency that facilitated the adoption. Obtaining Identifying Information: An adopted adult age 21 or older or a birth parent can provide a notarized statement granting consent or withholding consent to the release of information. If consent is given, then the information will be given to the requesting party. Any party may try to obtain identifying information by petitioning the court for compelling need. Providing Information: Supplemental information may be placed in the file by any of the following parties: birth parents, adopted adult, adoptive parents, or family of the adopted adult. Obtaining an Original Birth Certificate: An adoptee must petition the court in which the adoption was finalized. Contact: The Arizona Supreme Court. Posting Information
Welcome to the Search and Reunion Message Board!
You might also want to post on the SEARCH AND REUNION BY YEAR (of birth)and SEARCH AND REUNION BY STATE message boards for the best exposure for your search information. For ease in others reading these many messages it would help to post as a “new thread” and to include the following information in the subject line PLUS in the content of the message: gender of adopted person date of birth city of birth state of birth For example: male 01/01/1960 Chicago Illinois Other people reading the message board will be more likely to identify with this basic information and will be more likely to read your complete message and contact you if there is a match. Other great websites to check out: http://www.adoptionchat.com http://www.adoption.com http://www.adopting.org http://registry.adoption.com/ |
