Two names on birth certificate

I'm new at this. My son's father was adopted and my son wants to search for health reasons. He has his father's approval. His father just got his birth certificate. It says he was born Michael Henry Gorman also known as Michael Henry Bell. We're thinking maybe he was adopted by someone and returned. He remembers someone being burned. Has anyone had any experience with two names on their birth certificate? Thanks.

Gormen/Bell could be the unmarried birthparents last names...is this the Amended Cert or the Original?

I think it's the original but I would have to check. I need to get a copy. That's an idea, mother's/father's name. Thanks.

There are only a few states that allow the issuance of the OBC without legal proceedings...I wish I could remember which they were...I think Oregon is one of them...

Your sons father should also be able to contact the agency and get non-ID info...if the birthparents have updated it, then it should have health info on it.

He should also register with the Mutual Consent Registry in your state, if there is one.

I found out that he doesn't have the original or amended birth certificates. The information about the two names is on the adoption decree.

I see...

One of the names...is it his current last name?

If so, then the other would be his birth last name...

His adopted name was Richard Malcolm so both of those names were somehow tied to him at birth.

I'm not sure. He was adopted in Philadelphia, PA so maybe in PA. That's not for sure though.

Meaning of names...

Typically the source of names found on an original birth certificate could be associated with:

the birthmother's maiden surname

the birthfather's surname

the legal father's surname (husband of the birthmother but not the father of the child)

Another possibility for one of the names is that he could have been in foster care for a period of time. One of the names could be his birthname and the other the last name of his foster parents.
