For Adult Adoptees: You must be eighteen (18) years of age to request non-identifying information or to begin search/reunion services. Complete the Request for Adoption Archive Services form and mail or email to the address at the end of these instructions. Your adoptive parents will not be contacted. Any additional requests or instructions should be included on an attached sheet of paper. We shall correspond or talk only to you at the address and telephone/e-mail address that you provide.

I was born in Gainesville Florida on 12/25/70 Girl, My adoption was final in 9/71. Took a while to get B/f approval. I am searching for any birth family, I also would love the help of a search angle if one is out there that can help. Thanks

i was born 5/19/62 at st. joseph's hosp. and adopted on july 10, 1962 from catholic charities.-chicago, illinois. any info would help. my birth name was peggy ann.

ILLINOIS EXPANDS ABILITY OF ALL PARTIES TO USE INTERMEDIARY PROGRAM
Effective January 1, 2004, adopted people 21 and older and adoptive parents can obtain a court-appointed confidential intermediary without proving medical or psychological cause, as they had to under the old law in Illinois. Additionally, birth parents now will be permitted to participate in the intermediary program once the children they placed reach age 21. All parties can access the intermediary program for the purpose of exchanging medical information, obtaining identifying information or arranging contact with mutually consenting biological relatives. Previously, intermediaries could only seek medical information from biological relatives. Provided a "sought-after relative" has not filed a Denial of Information Exchange, the confidential intermediary will inform such relatives of the petitioner's request and of their options. The law also allows adult adoptees access to non-identifying information on original birth certificates and the "actual date and place of birth."

As of January 1, 2004 any adopted person registered with the IARMIE who was born in Illinois and there are indications their adoption occurred in another state or country can request that the name of the state or country where the adoption was finalized and if available the agency involved in the adoption be released to them.

Original Birth Certificate: An adoptee may receive through the registry or petition the court in which the adoption was finalized. To open the sealed Adoption file that the Illinois Dept. of Public Health/Div. of Vital Records have and obtain a copy of the original birth certificate -- a court order MUST be issued by the State and County where the adoption was finalized (many times the place of birth is different from the place of adoption) and it must direct the Department to open the file and release the record. White Male Born 7/4/1966 in Mecklenburg County Charlotte NC. Was adopted after 1 month stay in Mecklenburg Co. foster home. Foster father's name could have had the first name "steven" I think I was given his first name.

Looking for birth family

This is a GREAT idea! I was born in Pendleton, Oregon, 2/11/47 to Frank & Doris Adams and immediately adopted by Roy & Ruth Ann Bissinger. My name has always been Terry Ann. I have a brother, 2 1/2 years older whose name was Roger Adams, and a sister 1 1/2 years older whose name was Linda Adams, I believe. We heard she was later adopted in Portland, Oregon and re-named Gloria White. Doris & Frank divorced and she moved to California taking Roger. Not sure if my sister was adopted before Doris' move or afterwards...

Any information would be so appreciated! Thanks to all and good luck to everyone too!

I am an adoptee looking for any information... My adoption records list my name as Sarah, but I am not sure if that is the name I was given at birth, or through foster care??

I was born on 7/16/78,
at St. Charles Hopsital;
Port Jefferson, NY.

The only real information that I have to go on is that my birthmother was 16 when I was born, and had finished the 9th grade.
I have registered wit the NYstate registry- but that has led nowhere...This is getting to be a huge part of my life, that I can not live without answers to. Please anything would help..Also what are search angels? I have never seen this before- and I have been searching for over 6 years?

NEVADA ADOPTION REGISTRY
AND POST-ADOPTION SERVICES


The Division of Child and Family Services and Nevada licensed child placing agencies offer two important services to birth parents, adoptive parents, adoptees, and relatives related to adoptees within the third degree of consanguinity.

1. NEVADA ADOPTION REUNION REGISTRY

The Nevada Adoption Registry was developed in 1979 to allow adopted children and birth parents to locate each other if they both choose. Established as a mutual consent Registry, both the birth parent(s) and the adoptee must make application. The adopted child must be 18 years or older. The birth parent can be any age. Registry services can also be provided to relatives within the third degree of consanguinity as long as the birth parent gives written permission for the relative to receive information.

The Registry can also be used to provide non-identifying information about birth parents, adoptive parents, and adoptees. This request must be in writing. Examples of non-identifying information which can be exchanged are:

1. Child's birth information: Social / health history.

2. The age of the birth or adoptive parents at time of adoption.

3. Height, weight, complexion, eye and hair color, and ethnic background of the parents.

4. Education, religion and occupation of the parents.

5. Health of the parents.

6. In certain circumstances, specific medical information can be exchanged with the family's physician to help plan and treat an adoptive child. This is determined on a case by case basis and should be discussed with the Registry. Social history information can also be provided, if available.


An enrolled person in the Registry may withdraw his or her application at any time by writing the Adoption Registry. In order to keep applications current registrants must submit addresses and other changes as they occur.

In the event both the adoptee and birth parent and/or eligible relative files an application with the Registry, the Division will facilitate contact between eligible parties. If only one party registers, his or her application will be kept on file with the Registry.

2. POST-ADOPTION SERVICES

The Division provides information and referral services upon request to birth and adoptive parents and adoptees for assistance with adoption related issues.



Requesting an Original Birth Certificate: An adoptee must petition the court in which the adoption was finalized.
