Hello, I am looking for my fathers birth family. He was born in Santa Cruz California @ the old dominican Hospital on February 19, 1947. His birth paretns did not name him, and i heard his birth mother use to drive by his house and watch him from a distance.we dont have any names, so i am just hoping that maybe his mother/father, or siblings will try and find him and see this message. He was adopted by AL and Betty Metcalf of Soquel Ca. My brother looks just like my dad, and his son also looks like my dad. there is no mistaking that they are father and son. I enclosed a picture of my dad. His name is Thomas Metcalf

Your father can request this information from the adoption agency than handled his adoption. It will give him most or all of the following about the birth family:

A. Ages of birth parents at the time of relinquishment
B. Marital status
C. Other siblings born before him
D. Physical description
E. Ethnicity
F. Occupation
G. Education
H. Information about aunts, uncles and grandparents
I. Reason for relinquishment

***Waiver for Consent to Contact Form***
The Waiver is a form that can be used by adoptees or birthparents to give their permission for contact should a waiver be filed by both parties.

NOTE: If your father does not know what agency handled his adoption, or if he was a private adoption, he can contact this office for the above services:

He should have his request letter notarized before signing and mailing. It also helps to attach a copy of his driver's license and a copy of his amended birth certificate. Keep a copy of everything for your records in case you need to follow-up.

You might also have your father contact the Superior Court in Santa Cruz County and request his "Final Decree of Adoption". If he was a private adoption sometimes the birth name appears on this document.
