Question what is the difference between releasing .....

Identifying and non-identifying information???



i got 2 forms from the Indiana State Dept. of Health



i am looking for my biological parents and i have 2 forms to choose from. what is the difference?

i would rather her not know everything about me off the bat. but if i choose non-identifying info- how will she know to contact me? how would we meet or talk? i need some help from someone who knows for sure !!

requesting identifying will get you matched by the state... non-id will get you a physical description and insight into the situation that caused your relinquishment. If you still have questions, you might want to consider calling the state registry and asking them directly Your questions and concerns. You can not request BOTH ??
What state is this??

If you request non-identifying info, it will just give you general information and not give you names or where they live. In many cases, it doesn't give a lot of info.

If you request identifying info, you only get it if your birthmother or birthfather have registered, looking for you. Then they'll match you up and give you each others name. But if only your mother has registered, you'd only get her name and address. Not your father's. He would have to register separately.

You could try to get both. If you only get the non-identifying, then you could use the confidential intermediary program if you wanted to speed things up a little. Indiana has that program.
