sample petition to unseal adoption record

I am trying to unseal my adoption records in Nevada. The statutes state:

The files and records of the court in adoption proceedings are not open to inspection by any person except upon an order of the court expressly so permitting pursuant to a petition setting forth the reasons therefor

I am wondering if anyone has an actual petition used for this purpose (any state will work). Basically, I need to know how to format the petition so it will comply with legal protocol.

I could use this information as well. I was born and adopted in NY and I'm not even sure how to begin. I want to request information from my adoption agency, as well as from the state courts.

Thanks BrandyHagz -- this should work nicely. Now i need 1 more thing that i should have mentioned in my original post.
The county clerk tells me that I also need to submit a document that the judge can sign if he approves the unsealing. Hopefully someone on the forum has actually unsealed their documents and can post a copy of the original order from the judge telling the county clerk (or vital records) to allow them to view and get copies of the records (with their names covered of course)

Where can I find copies of those petitions that I can download off of the internet? I am hoping that I could find the petition that I need for my own search.

Hi there,
I am jsut now starting my search for my birth mother and I have no idea where to start. I have no info. on any of my birth family. I was born Dec. 16, 1973 at Alachua General Hospital in Gainesville, Florida. Any help anyone can give me is greatly appreciated.

You need to go to the probabte court in your area. My records were sealed and I was able to get them unsealed. You have to fill out a form then it goes to the records department and then they pull out your file. If your birth family was looking for you there will be a paper in there stating it is ok to unseal the records. The process takes a very long time. Took about 2 months for me.

Another thing you can do is if you know when and where you were born you can call the hospital and see about possibly getting a list of births on the day you were born. In my case I was the only baby girl born that day so it was pretty much obivious. Well if anyone needs advice or help feel free to send me a message. I wish you all luck in searching for your birth familes.

I called the hospital where I was born and because of the hippa law they wont give me any info.
Where can I get one of those forms to unseal my records?
The sad thing is I work for this hospital and I thought that I might have some pull to get that info. but I don't.
If any one is out there that can help me I am putting in my email address.

Welcome to the Search and Reunion Message Board!

To request a copy of the petition form to open your sealed adoption file contact the court where your adoption was finalized. They may not have a form on-line so ask them to mail you the form if necessary. After filling out the form attach a copy of your driver's license and a copy of your amended birth certificate. Write a cover letter explaining your reason for requesting that your file be opened. Read the instructions for filling out the form for the reasons that the court will find acceptable for opening your file. Have your signature on the petition notarized before signing and mailing. Keep a copy for your records.

I'm a aunt looking for my nephew, and on one of the replys it said that when the adoptee goes to get their files opened there will be something in there stating that their family is looking for them and therefore it will be ok to open the sealed document. My question is how do I go about letting the courts know that we are looking for John and to put that in my nephews file? Thank you all for your time, bre36

A great site to go to for sample letters and forms is www.birthfamilies.com A good letter to send the adoption agency if known is letter of confidentiality so if birth family is ever looking and contacts adoption agency the letter is in your file to okay the contact.

I'm starting my search in the state of Texas and live in Austin where the Vital Statistic buerau is kept. Funny thing......I used to work there. Frustrating to be feet away form your life and not be able to find out. Do you know of any forms for Texas?

Petition to unseal records

I was born and adopted in Ohio in 1974. According to our laws I can only request and receive non identifying information. Should I try to petition the court to release my full records? If so, do you have sample petition form for Ohio?

Contact the court in the county in Ohio where your adoption was finalized and ask for their petition form. In many states each county uses their own county form rather than a general statewide form.
