I didn't think I would be so confused, but I am so I'm turning to the people I KNOW have the answers. We need to renew our passports because they are about to expire. For DS do I need to complete the DS-11 form and send in his passport? For us is it the DS-82 form? I'm not sure what to do with DS, I guess that's my biggest concern. Thanks!

I don't have the answer, but I would go to your "County Clerks" office and ask them. At least, in NJ, your County Clerk is the one that processes (forms) for passports, and then they get mailed back to you from the "PassPort Gods" (heheheh), mine came back from Atlanta, GA.... go figure that one out
![]() I had mega questions when I filled out my forms (never had done it before), and they where great!!! Just a thought, especially if you where not to far from the County clerks office.....

To renew your child's passport it is at different form and you can't do it via mail unlike renewing your passport.
Both parents need to be present or one has to sign an affidavit stating they are aware of the passport being renewed. I guess this is to prevent a child being taken out of country without the other parent knowing. We had to renew our oldest son's Russian and US passort this year to enable him to come to Russia with us. I got this for the state department's website. 1. Both parents must appear together and sign or 2. One parent appears, signs, and submits second parent's notarized statement of consent authorizing passport issuance for the child (a notarized Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 14 , or a notarized written statement with the same information on a sheet of paper from the non-appearing parent) Here is the link to read more about it.
