Post Adoption Communication Agreement

I want to find a form or a template for a post adoption communication agreement. I am a birth mom and my husband is adopting my 4 year old daughter. My ex-husband has signed his rights over, but we really want in writing the openess and guidelines to reassure her birth father and his family that they will not be cut from her life. Please help! I have gone this far with no legal aid, just getting through the documents and submitting was a big deal, now I want to make sure its complete and encompasses the concerns of the family as well.
