Who gives the diagnosis?

Who gives the diagnosis for bipolar or other mental health conditions? I feel as if we are just spinning our wheels and getting no where with L. His mood swings are exhausting (he has had several “down” days which just break my heart and he has had a few days lately where he is just cycling nonstop from high to low multiple times a day). Psychologist is trying to help come up with a program to stop L from wetting and soiling himself. We’re doing EVERYTHING they ask and it is getting us no progress. L now will remove his pants and defecate on the toys or in someone’s bed. He often rages when sent to time out – and these rages of screaming and pounding on the walls last over an hour at a time. The meds he is given for ADHD work about a third of the time (the other 2/3 are pretty much evenly split between him being “down” and comatose or him being “high” and so active that getting run over by a bus wouldn’t slow him down). I just don’t get it. I feel that I have lost my son to a beast that we cannot pinpoint or name. And to top it all off our psychiatrist is out of town so we had to see a resident for our monthly appt. She was very dismissive and said that his problems were all behavioral and was very flippant with me saying “I know you have your hands full with all the other kids.” I just want to SCREAM at all the “professionals” involved and tell them that we are dealing with my SON, not just a chart or notes from the preschool teacher who is 100% anti-medication no matter what the issue is!!!!!!!
What have any of you done when faced with this type of brick wall?
