Affidavit of Support
For any of you who stayed in Poland instead of making two trips. Did you have to provide to someone a notarized affidavit of support form?
I'm assuming this is something stating that you can support yourself while in Poland and are thus not going to be a burden to the country. But - where can I get this form, and to whom does it go? Thank you in Advance!

I stayed and was in Poland for six weeks and did not need anything like that. No one asked for it or questioned me.

We were there 2x for 7 weeks each time...never needed an affidavit of support form.

Figured it Out
This form simply states that you are aware of the child's known or possible medical issues and that you are knowingly taking them none-the-less. It seems to be signed in Warsaw at the end of what's usually the second trip when you're dealing with medical appointments and passports, etc.

OK, yeah I had something like that to be signed, I think for the embassy or something. I had to say I had noticed my kid had differences. LOL

That is hilarious - did they think that perhaps you weren't that observant? So glad we can laugh about this stuff!

"Have differences" form
We were told our judge required us to sign, notarize, and fed ex such a form asap before she would grant us a court date. The Fed Ex cost was $90 for one slip of paper.

We didn't have to do that either. I remember the clerk at the American Embassy in Warsaw asking questions about our kids - what is their level of intelligence, health problems, why were parental rights terminated, ... And "Do they look normal?" I think it was worded like that, or something equally offensive. I wondered what happens if you tell the clerk no, or what if she sees the kids and tells you "those just aren't normal looking kids, sorry."

Our Embassy clerk was very appologetic about the "are they normal in appearance" question and said it was archiac and found it just plain stupid - but she had to ask it anyway.
I don't remember doing anything like that either - and know we definitely didn't fed ex anything like that. I'm surprised our "fun" judge didn't require it!
