adult adoption

I have been close to this man 33 with 3 children for 10 years we have considered each other father and son and am one of his children godparrent.I plan on leaving my home to him when I die I am 65 and would like to adopt in order to avoid legal issues with rest of family but we dont want a name change due to his children.
would appreciate any advice I am single with no children.
