We sent our I-600a application in last Saturday and received it back today. There is a problem with my name that they need "documentation" for. They say I must use my birth name on the application. For example, my maiden name was "Lori Sue Smith" and I always went by "Sue", so when I got married I dropped the "Lori", had it legally changed and my social security card now reads "Sue Smith Anderson." Has this happened to anyone else? Any idea on what kind of documentation they might require? I'm hoping my social security card will do. The good news is that at least I know the app is not sitting for too long!

How about with your social security card a certified copy of your marriage certificate.

We had to have dh's name legally changed, in court - his bc said Francesco and his passport said Frank. It was fairly quick, you can download forms from the internet, then get fingerprinted (yes, again) then take everyhting to your local courthouse and get a court date, but it costs $256 and now we have had to send his passport away to be amended!!!!
