Replacement COC
Hi All!
I am attempting to complete form N-565 to change my sons name on his COC. I did search the forum, and found answers to many of my questions. There are 2 things I can't seem to figure out. 1)What to list as the USCIS Office that issued his original COC. I did read that Buffalo is the only one that issues them, but I'm not sure if this is true. 2)Where and how to sign. I have read many different things about where to sign the form. Part 7 or 8 or both, and do I put mother after it? I tried to call the 800#, but I'm pretty sure there is no way to get a live person on the phone!! Oh please let this be the end of the paperwork!! We're working on this right now. I made my best guess at the signature part and it got returned for incorrect signature. So then I sent back the form with 1 front page, 4 versions of the 2nd page, and a note telling them to pick the best format. A few weeks later I got a note saying it was under process, but I don't know which version was approved.

Also--doesn't the COC list the issuing office on it somewhere? I thought I had that information from the document. Mine is currently in their hands and our copy is at home, so I can't look to see. Which reminds me MAKE A COPY OF YOUR COC BEFORE YOU MAIL IN THE ORIGINAL. Just in case.

I did see your post in my search. I'm glad to hear that at least one of them worked!! That is probably what I'll do if I don't get any more replies. I did just look over the COC and I don't see an issuing location listed anywhere - just Department of Homeland Security. Thanks again for your help!! Michelle |

Our N-565 got sent back because I only signed on #8. I then signed on #7 also and sent all the paperwork back. I have since received a formal document saying that my paperwork has been received.

All COC's issued for an IR3 visa (as opposed to replacment ones for name changes) are issued by the Buffalo office. I don't think it says so on the COC itself. (I am not at home at the moment and can't check) but only on the envelope it came in.
