Hi I havent been on in ages, but i have a question. What is the name of the form we need to fill out to get our "originals" back from INS??? Hope yall can help me. Thanks!!

USCIS Home Page, the form G-884 for the envelope upon entry returned. Form G-639 returns all over documents used in the adoption. If you need instructions on either you can email me and I'll forward them to you.

Are we both the requester and the consenter on the G-639?

I'm also wondering what's the difference between an Alien Registration Number (they call it an 'A' number) and the naturalization certificate number? (I visited before pickup.)

Not sure about the G639 requester and consenter question. Kelly (Cbrosa7) will know.

The A# they want is the INS registration number on the COC or on your child's visa in their Guat passport. This is your child's assigned number that their file is tracked by. The other number at the top of the COC is the certificate number. Now why they both have to start with an 'A' I have no idea! One of the many mysteries of USCIS!

we did the G664, and got back all of the forms (I think???)but I really wanted the monthly'well visit ' reports from the dr.- ht,wt, development,etc...is that another form?

2 forms, G884 is the form to return the docs in the envelope when you entered. G639 returns the rest of the documents in the file. Everyone's file can be different, it may or may not be in there and you won't know until your file. G639 will take close to a year to get back usually.

Just noticed you said "monthly"
Your monthly well visits would only be included if the government had them for some reason. Usually that comes from your agency. I would think the only medical from the government report would be the final or DNA related.

oh,so if , while in process, my agency didn't give me much of the monthly visit info, I probably won't get it? I got some reports of her well visits, but not reguarly, and I wanted to put together some kind of lifebook with this info in it.......

I would be very surprised if things like well child check info. was in either. There is some variation in what you get back from the G884 (the form that gets back the sealed packet you turned into Immigration) but it is all official paperwork the Embassy had from the adoption - Family Court Report, PGN decree, DNA report, that sort of thing. You might get more back from the G639 as well as that gets anything that the govt. is holding, but like Kelly said, copies of well child reports would only be there if the govt. had reason to have needed them at one time.

LOL! Well, USCIS is like Forms-R-Us!! Since you have done the G884, you might fill out the G639 just to see what you get. Since you are looking for forms to fill out. Not likely to get what you are looking for, but you never know what you might get!
