Hi All,
Can anyone help me fill this form out. Am I suppose to fill one out for me and Wade. Whose family name do you start with your husbands or yours. I'm primary petitioner etc, etc, Any help would be greatly appreciated. I don't want to screw this up. Also, thanks for all the well wishes they mean a lot to us. ![]()

He's getting so big ![]() 3/2 Back into PGN ![]() 3/25 OUT of PGN. Thank God ![]() 5/28 Received BC from Sayachen, Peten ![]() 6/22 Pink received 7/1 Embassy appointment 7/2 Visa 7/3 On plane back to USA

I have a copy of this at home from when we went--I'll look it up tonight once I get home and if I no one else responds by then I'll tell you what we did.
My agency had a great and very detailed instructions in their website but apparently my account was disabled sometime since he came home. Hmph. ETA: Just wanted to add: SOOOOOO HAppy for you both to be having this "problem" finally!!!!

(from the instructions I have printed with my sheet) Fill this form out as if the child was filling it out. All answers are pertinent to the child. Always use the child’s birth first and middle names. Your child’s name does not change until he/she is legally readopted in the United States after you come home. Part I 1. New Last Name (your last name) followed by Birth first and middle names. 2. Child's full birth name. 3. N/A or blank. 4. Date of birth (mm-dd-yyyy). 5. Child's age (if your child is a baby, write the word "months" leaving space to fill in the number when you travel as it may change). 6. Place of birth: If you look at your child's birth certificate or power of attorney, the municipality=city and the department=province. 7. Guatemalan. 8. Gender: check appropriate box. 9. Single, never married. 10. Name of petitioner, Home address in the United States, phone number. 11. Unless there is a different mailing address, leave blank. 12. N/A 13. Hotel Address where you are staying in Guatemala. 14. N/A 15. Adoptive father 16. Adoptive father's date of birth, place of birth (city, state) and current address. 17. Adoptive mother, use maiden name (do not put married name anywhere on this question). 18. Adoptive mother's date of birth, place of birth (city, state) and current address. 19. N/A 20. N/A 21. I'm glad to see that I did it right. I was so nervous I don't want to screw anything up. Claire not that the questionaire has changed a bit so don't follow the numbers exactley has Julie has them. You can figure it out easily enough by reading the answers she gave.
