another G884 question or 2

Can anyone answer a few questions about completing the G884?

1. Part 2 lists family name, given name, etc. Do I put DD's original name (or adoted name she came home with)? any other names would be name on Guat birth certificate. Obviously, name on COC is her readoption name. Is this right?

2. Also, A number refers to the CIS registration number. Certificate number is in the top right (also an A number). RIGHT?

3. Finally, is the Buffalo address correct? I cannot find where to mail it anywhere. I've read some archived threads and this seems to have changed. Anyone?

I so hope to get a good copy of the birth mother.

ok, last one really

as far as documentation, i was going to send my drivers license and my passport; DD's guat passport, US passport, COC and readoption birth certificate. the BC has my name on it. is that enough?

1. You need to put the name she came home with (first, middle, your last name). any other names used would be original name (first, middle, Birth last name)
Readoption name would be the name if you have changed her name already (first, middle, or last)

2. You can verify your A# by looking at both the COC and the passport/visa. The correct number will be the one that is on both.

3. You need to email uscis with your child's name on entry and there A#. Ask them to locate your file and where to send the form. email is: Child-Citizenship-Act@dhs.gov


Documentation:
you need to send a copy of child's passport.
copy of BC listing you as parents or Copy of COC

To add to Kelly's answer . . .

Yes, the A# on the COC that you want is the CIS registation number. The other is the COC number.

For supporting documents, you need two forms fof ID for the child (typically a copy of their passport and COC), a copy of the BC showing you as the parents so show you have a right to the information, and some people are asked for ID for themselves as well, so personally I would include two forms of ID for yourself as well. (Copy of DL and passport.)

I have also seen the directions interpreted (by someone in USCIS) that when it asks for two forms of ID, that it is for the person requesting the documents (you) not for the person you are requesting them for (your child.) Which is another reason to include two forms of ID for both you and your child

The reoccuring issue with USCIS is that there can be multiple 'correct' answers depending on who in USCIS is accepting the form.

Can I build on this...? I'm so confused by this form!

photo copies we'll be sending because I'm covering every possible base:

1. My DL
2. MY US Passport
3. NY "certificate of birth data"
4. copy of name change order to connect #3 to rest
5. J's COC
6. J's Guatemalan & US passport

(note: 4 & 5 both show entry name; his US passport shows adopted name)

Now...to the actual form; Part 3.
What is up with the

"I [] swear [] affirm that I am the person named in part 1..."

Am I supposed to choose one? What's the difference? And I get it notarized, right? And then do I just leave the other part of that blank where the USCIS official says I personally appeared (which I am not planning to do)? And does my notary fill out the bottom half as "certifying official"?

You can swear or affirm. Your choice. Someone explained it to me once and I wish I had saved the explanation, but it had something to do with some religions can not swear to things. So there is also the option to affirm.

Then you have it notarized and the notary fills out the certifying official portion. Skip the USCIS portion unless you are filing it in person.

There is some continuing discussion regarding how to interpret the documentation required with the form. Some read it as two forms of ID for the adult filing out the form and a copy of the BC showing your relationship to the person (child) you are requesting the info. for. Others see it as two forms of ID for the person the info. is requested for (the child) and a copy of the BC showing that you as the parent have a right to that info. Both methods have been successful. And some people just send it all to cover all the bases. (Which in my opinion makes the most sense as you just never know with USCIS!)

No! Do not send an origional anything with the G884!

Copies only as they will not return the documents you submit as evidence. I would hate to see you loose all those important documents into the USCIS system!

No, I'm sending a copy.

First because the file "checkbox" area says "did you attach a copy of your identity documents" so it's all photo copies for us.

Secondly, because our original COC went out today in the mail with our update forms to get a new one in his new name. So all we have are copies. Luckily I thought to make a copy before sending it off so I will make a copy of my copy and send that with the 884.

There is no charge for the G884. (Something free from USCIS!) If you email the Buffalo office at BUF-G884@DHS.GOV with your child's A# (on the COC and on the visa in their Guat passport) they can tell you where the packet is located and you can be sure you are sending the G884 to the right office.

There are two USCIS Service Centers that process the N565. If you look up the instructions with the N565 they list which states send it to the Texas Service Center and which go to Nebraska.

ETA: You are in Michigan aren't you? For Michigan the N565 goes to the Nebraska Service Center

It says Specific information about desired documents or records. WHat all did you write in this area? I want to make sure I get everything, especially Pictures if there are any.

Isn't there a place on this forum that we can get the forms? I swear I got the forms off here before, but I have messed them all up
