I'm a little confused by this application. Grace has been home 2 years now, it's about time I got around to filing the name change on the CoC (ir3, readopt done).

My questions are: how do I know what office issued the certificate?

Do I need to put her birth name for 'other names used', because she's never used that name in this country.

Which one's the certificate # and which one's the A#?

And do you sign for the child in #7 or sign #8 or both?

I know I've dealt with the A# before, but I still can't remember if it's the one in red or the one next "CJS Registration No."

The Buffalo office issues all IR3 COC's.

Yes. I would put it on there.

The A# will say INS Registration Number or something similar next to it. The other is the COC number.

And do you sign for the child in #7 or sign #8 or both?

I personally signed in #7, but I honestly dont' think it matter much.
