G 639- Instructions?
I need help figuring out the G 639. Does anyone have instructions? Thanks in advance!

CT4 11-1-06 signed w/ agency  11-10-06 began homestudy  3-17-07 baby girl born  3-26-07 received referral  6-13/6-18 1st visit trip  6-25-07 DNA received 9-6/9-9 2nd visit trip 9-9-07 Pre approval 9-14-07 Entered PGN 11-7 K/O of PGN  11-13 resubmit 11-21/11-25 3rd visit trip 12-23-07 100 days in PGN  1-19-08 4th visit trip KO'd AGAIN!!!! 3-4-08 RESUBMITTED!! 3-17-08 1st Bday-5th visit  5-9-08 birthmom interview(Not really, actually 5-28) 6-6 6th visit trip 6-24-08 OUT!!!! 9/18 Retalhuleu BC 9/24 Passport 9/26 2nd DNA 10/17/08 HOME!!!
