I'm completing the G-639 form and I had just 3 questions on it and could use some help, please.

1). For the first part that is the type of request, do I fill out FOIA or Privacy Act? I want all the copies of whatever is in the file so which one do you choose to get everything?

2). On part #5 where it mentions the Naturalization Certificate, what is that? We have a COC. Do I put that there instead of the Nat. Cert. info?

3). Do you send some kind of ID to show proof of who you are, etc?

Vicki,

1.) FOIA
2.) Only the COC info pertains to you.
3.) Yes, you need to send proof for the child as well as your ID proving parentage. I'd send copy child's BC, copy of passport ( both of you).
