Hoping someone knows the answer to this...

I have a CoC for Hannah however it is in her Birthname with our last name. I started to complete a N-600 and apply for a new one. Then I saw the form N-565 which says it can be used if you already have a CoC but your name has been changed by court order.

As part of her US readoption I have a court order for her name change.

Doesn't it make sense that I can use the N-565 and just request a new copy with her updated name. I know it seems obvious that I could but I am not always sure things are so obvious when it comes to our govt.

Yes, for an updated COC (name change) you use the form N-565.
The other form is for IR-4 visa, new COC.

Yes, you are correct. Use N-565 - Application for Replacement Naturalization/Citizenship Document. Plus it's easier to complete and shorter than the N-600.

OK so here is another question....

It appears that I have all of the original docs. The ones I have the official stamps plus the English translation attached as well. I have the POA, SW Report, Adoption Decree. I have one picture of Hannah's birth mom that came with her DNA results.

Am I missing any other originals docs?

Are you asking about this in relation to filing the G884? And whether there would be more in the packet than you already have?


If so, we got quite a few thing beyond those. I have to be out the door in 5 minutes, but when I get back I can post a list of what was in my kids' packets.

If you are asking about the G-884, everyone's packet is different. Mine had medical reports I hadn't seen, some have additional Birth mom pics, the Birth Mom's Cedula, additional interviews. It will just depend on what is in your file.

I must have looked at it wrong when I checked earlier I was thinking it cost $320.00 but I see now that it is in fact free so I will for sure file it.

Does it matter when you file for it or should I wait until I get the corrected COC?

On a side note: It has been a long time since I have posted.. I read alot but all of your help and replies have reminded me of why I joined this forum in the first place!!! Thanks again!!!

Melanie,
No cost to file G-884 or the G-639, and you should do both. I have samples and step by step directions for both should you need them. Email me and I can forward the info to you.
You can file at anytime. For G-884,You will need to email USCIS at:
Child-Citizenship-Act@dhs.gov
send child's name on entry and the A# from your COC, also on the passport/visa. Ask them who currently has your file and where you should mail to. They usually respond in 24 hours.
