We are having a hard time filling out this form.
Can someone tell me what is the certificate of naturalization? Also, what is the family name? Is that her original last name? Is the Given name her new last name?

Also, we have not received her COC yet. I thought I read that you don't necessarily need it to get the G-884. If that is true what do I put in the C of C # box.

Also, is the permanent Resident Card that same as the green card?

I would appreciate any help I could get.

Just put N/A in the boxes that refer to Certificate of Naturalization. (My understanding is that you are 'Naturalized' and get a Certificate of Naturalization instead of a COC if you become a citizen after the age of 18.)
And since you do not have the COC yet, also in the COC boxes.

Family name is her last name. Then her first name in the Given Name box and then Middle Name. Then in the Other Names Used box, put in her birth name.

Also, is the permanent Resident Card that same as the green card?

Yes

Not sure if you are having trouble with any of these, but these are the other questions I see here a lot . . .

Port of entry is the city you entered the US when she came home. (Where you went through Immigration.)

Type of entry is the visa type. (IR4 it sounds like in your case.)

The A# is on the visa in her passport. This is the number the file is tracked by. If you can't find it let me know and I'll pull my kids passports to see exactly where it is on the visa. If I am remembering correctly, it is called the 'registration number' on the passport, but I would have to check to be sure.

You need to have the form notarized and you need to include two pieces of ID for you child (copy of Green Card and passport) and proof of your relationship to the child (copy of BC.) Many people have also been asked for two forms of ID for themself even though the directions don't ask for it. Personally I would put in a copy of your driver's license and passport just to cover the bases.

The G884 needs to mailed to the last office that took action on the case. It sounds like in your case that would be the office that issued the Green Card.

You can go to legal-eaze.com and click on the adoption information for a sample G-884 filled out. You will need the A# from the C of C to file and you will need to email (child's name and A#) to see who currently has your file before mailing. "A-Number" stands for Alien Registration Number. It is on your child's foreign passport, and also on his/her green card.

Naturalization certificate means COC.
