I have a couple questions about pick up paperwork:


the immunization forms says it needs to be notarized. Do we each (DH and I) need to fill one out and sign it, notarize it, or just one of us?

the I864W: again, do we both need to fill one out and sign/notarize it or is it just DH (he is listed as the petitioner on the I600)??

how do we certify our tax returns??

I had to go back and check but on the vaccination affidavit, I simply put the following:

We, Amanda X X and Janes X X, certify .....


Then at the end I had a signature spot for each of us:


________________________
Signature Adoptive Mother


________________________
Signature Adoptive Father


Then our notary did her part that said:

Subscribed and sworn to before me on this 12th day of October, 2007, by Amanda X X and James X X, who are personally known to me or proved to me on the basis of satisfactory evidence to be the person who appeared before me

Only the primary petitioner for the immuniz.

For the I-864-W we also did only the primary petitioner (me).

The DS-230 you fill out but don't sign or date at all until you're at the embassy interview.

For the taxes, my understanding is just a "we hereby swear that these are a true and accurate copy of the original" type statement and then "signed before me this day..." notary language. Do that on a cover page and staple it all together. But if you've visited and are on the "W" form, you don't need them. They'll look at your passports and check the date versus your PGN approval and that's it. We had ours with us just in case but didn't need them.

OK I just checked and I used my name on the I-864W because I was the primary petitioner. Hope this helps and good luck!
