Documents needed for revised CoC??
For those of you that have gotten a revised CoC for your children, what documents do you need to send with the N-565 form? Here's what I think, but would like confirmation:
- copy CoC in old name (or original?) - copy of readoption decree showing name change and me as the parent - 2 passport size photos Am I missing anything? Thanks- Krista

You do need to send in the origional COC, not just a copy. And then the name change documents and the pictures like you mentioned. And of course a check for an absurd amount of money.
 On the positive side, you are almost done with paperwork!!
