I'm not from Michigan, but your state shouldn't matter. There's a link at the top of the page for the passport website, and they tell you exactly what you need to submit. It's pretty straightforward.

We have done 3 passports including Frankie's who got his passport 3 months ago.

We sent: Michigan birth certificate, COC, Court Orders for the Name change, PGN Protocol, Original birth certificate.

We have not had any problems.... if you are having a problem...maybe try changing offices... I know our local post office knows me by name...and they are super helpfull!
