FMLA - Certificate of Health Care Provider

Hi Everyone,

I have been asked by my employer to have my doctor fill out a Certificate of Health Care Provide Form in order to qualify for FMLA. However, all of the questions are very specific to long-term care and disability.

Has anyone else had to have this form completed by their doctor? If so, can you provide me any insight. My doctor is at a loss, and so am I!

Your child's doctor fills it out - there is a part that refers to having to care for someone - and you are caring for the child. Have you picked out a Pediatrician yet?

I administer FMLA for my company. It should be a standard Dept. of Labor form that the Dr. fills out for FMLA. There are 2 types. One for yourself and one if you are caring for another individual. If your having to fill out one to go on "adoption leave" then it really doesn't apply. The company would ususally provide a request form to you and that would ask you what type of leave you were going out for ..one being adoption of a child. So the medical questions on the Dept. of labor form wouldn't apply.

My organization gave me 4 forms to fill out for FMLA. When they found out it was for adoption, I only had to fill out the internal leave form and provide a letter from the Adoption Agency. Hope that helps. Good luck!

I second what bbeard said -- the doctor portion is just if you are sick or someone in family is sick. The form is probably a WH-380 (it has title you quoted above). Most people think of that as FMLA form but you can see if there is another form attached that talks about adoption or just put in a block on the WH-380 that you are taking FMLA due to placement of a child with you for adoption so the rest of the form does not apply.
