Name change form
Help!
Does anyone have a generic name change form? We would really appreciate it. We easily added a name to our childs with a simple paternity form signed by a judge in order to get a state birth certificate, but don't think 'USCIS will accept that as a court decree and we need to ammend his Cirtificate of Citizenship to reflect his new name. Has anyone had to ammend the name on their childs Certificate of Citizenship? If so, what did you provide for proof of name change? Thanks, Mamalama |

In Missouri, they don't do a court order either. I just had to apply for a new birth certificate and they did the name change at the same time. I did have to supply a copy of DD's Guate BC as well as the original PGN decree and translation.
When I applied for her new COC, I just attached her original BC, the new BC and put a note that Missouri doesn't require a court order to change name. After about 2 weeks I received a receipt and my check was cashed by USCIS and I haven't received any request or denial yet so I am hoping that they will accept the paperwork that I sent. I doubt that they would accept a "generic name change form" since that would be very unofficial. Good luck. Susan

When we did our children's name change, all we received was a copy of the letter from the court to the office of vital records asking them to issue a delayed registration of foreign birth in the kids' new names. No where did it mention their old names or that a name change was done. USCIS accepted that and we had their new COC's in under 3 months.
I honestly would send a copy of what you have and I bet you will be fine. Good luck!
