Need advice ASAP on N600 (Application for COC)

So the background info is this. We completed our daughter's readoption in May. We have gotten her birth certificate, social security card, and passport. The question's I am having problems with are this:

When did your daughter become a U.S. citizen? I am pretty sure it was the date of her readoption since we hadn't visited before we brought her home. But then it asks what USCIS office granted the change of status. I don't know how to answer that one.

Also it asks if we have ever applied for a U.S. passport or coc before. I would assume the answer to this was yes except it says, "if the answer is yes attach explanation" So I am thinking this means if you applied and were rejected but that's not what it says.

Need advice from all you experts ASAP as I need to get this in the mail today. Thanks!!!

She became a US citizen the day the readoption was complete. The second part of your question, about the USCIS office (Part 3, #5, D) is not applicable to you.

About the passport one, if she has a US passport, put yes and on the separate piece of paper put that she has obtained her US passport.
