Replacement COC

Anyone who has returned the certicificate of citizenship in order to get a new one in the childs new name -

Did you sign the COC that you returned?

I did not. It was never signed. I haven't heard anything back about the new one yet, but I haven't received a request to sign the old one either.

I did sign the original COC. The instructions that came with it had instructions to sign it and sign on the edges of the picture also.

Can you please tell me how you signed?

Did you Print the childs name and then sign your name and write mother in parenthesis like they ask on the passport?

How long have they had it?

Did you write your childs original birth name in the field that asks about "other names used?"

I signed "my name for DS's name" and also signed his picture as the instructions requested but that was last October - as we all know instructions and protocol are constantly changing.
