New COC
Does anyone know the proper form to use for this? Is it the N-565 or the N-600? I am hearing conflicting things for this. Also, does anyone know what the new fee is? Is it still $220 or has it gone up? Do you have to send the original COC along with the form? Thanks!

It looks like your child came home on an IR3, so you will use form N565 to change their name on the COC. It is form N600 if you are applying for a COC for the first time (IR4 visa.)
The fee goes up on July 30th, so you want to get on it right away. You do need to send in the origional COC with form N565. I made several color copies first, just in case anything happened to it. If you have any questions about the form, feel free PM me or post them.

Thanks a bunch Debbie. Do you, or anyone else, know where to send the form to request your original documents? I live in Kentucky and I went on-line and it looks like I maybe send it to Texas, but not sure. And I'm not sure of the address either. If anyone knows I could use some help here. Thanks again!

Also, do you have to send new passport photos? My son has been home six months. Ughhh...this never ends!!

Last question...I think! Am I sending for a new COC and request for return of original documents to the same USCIS office? Any idea where? I live in KY. This is almost more confusing than the original adoption paperwork!! Thanks!

Just in case . . . Your G884 goes to the Buffalo NY office that sent you the COC. For Kentucky the N565 goes to the Texas Service Center. (There are Service Centers in Texas and Nebraska and which one it goes to depends on your state.) You will need new photos to send with the N565.
Aren't these forms horrible?! All about as clear as mud!
