Question Child Care daily report

My DD is being cared for by a family friend. She doesn't run a daycare, just agreed to watch my DD. She also watches my friend's DD 1-2 mornings a week. She has children of her own that of course are home during the summer.

My question is that I have heard some of you mention that you get a "daily report card" from your child's daycare provider if they are at a center.

I would like to make up my own daily report card to have my sitter fill out to help me feel more informed about what is going on during the day.

Can any of you tell me what type of information is on the reports that you get?

Thanks so much for any help you can give me. This has been a hard transition for me to not know what is going on during her day.

The center that I work at calls them "tattlers"-

The Infant "tattler" shows- fluid, solid, and diaper changes (bowel movements are of two choices- loose or firm can be circled)and sleeping times. There is a time line across the top of the page and all info is listed under the time it is given. Each child has their own "tattler" for each day. All parents are required to tell us at drop off when their baby woke up, last diaper change, and when they ate or had a bottle.

The Toddler "tattler" has a space for lunch, potty/ diaper changes with the abbreviations W(wet) D(dry)
S(sat on potty) and a star symbol (for when they actually produce something on the potty!!) Nap time is recorded, along with Lunch and any activities your child enjoyed through out the day! ALso the child's demeanor is recorded ie: playful, good helper, friendly, etc...

My friend keeps a small notebook in her diaper bag that she and her care giver communicate with.

My DD's daily report when she was in the infant room (for about 3 weeks) consisted of when she ate, how much she ate, every diaper change and what they changed, as well as any health concerns. It also included her naps.

Now that she is in the one year old room the report differs slightly. It includes diaper changes, the actual time she fell asleep and when she woke (which I really like because it gives me a good idea of how the rest of her day is going to go), if she ate all, most, or some of her lunch (I have to pack her lunch so I know what she got), the activities they did that day (songs, outdoor games, arts and crafts), health concerns, behavior challenges, and gernally how her day went.

I like it and I can't think of anything I would add to it at this point. It's nice to get a little something each day. I can talk to her about it and it makes me feel like we're connected even when we're apart.

Our nanny has a chart with the hours in care across what is sort of the x axis, and lines for food, drink, output, and naps on the y axis. She will mark the start and end of the nap, time and content of diapers, and rough idea of intake (how many ounces of milk? Did he finish his lentils?). She also has an "other" column....we have her record meds there if he is taking them, or sometimes she records activities outside the home, or comments if there is a bump or scrape or other unusual occurence. She also notes there when I need to bring more wipes/diapers/cereal/etc (we share the nanny in another person's house....)
You could type up a draft and have her try it....we modified it a couple times before we got it to a place where we were both happy!

Ours dcc records diaper changes (wet/dry/birthmom, etc.) and time, times and what he drank/ate for breakfast, lunch, and snacks, when he went down and woke up from naps, and when he was much smaller, times he got "tummy time." There is also a notation area for medications if applicable as well as a comment section to tell me of anything paticular that happened in his day (i.e. he really like playing with a balloon today or with bubbles) or simply to tell me that he needs diapers or wipes or formula (which he just came off of). I also like to talk to and get to know the people taking care of my ds so they can feel comfortable telling me about his day as well. I read his daily report every day.

We get a sheet everyday that at the top says "Chloe's Day".
there is a category for naps and what time she took it, snack and the time, lunch and the time, and she is potty trained but they still put the time down every time they take her to the potty and whether she was still dry or soiled. This is also my way of knowing if she ate good because they will notate for instance if I sent something in her lunch box that she really liked and ate alot of (so I know to send it again) or if she wouldn't touch it (I know to not send it).
And there is a section for notes so that is where they will write to let me know she needs more pull ups or wipes.
It is a great way for us working moms to stay informed!
