I have my Embassy appointment this Friday. I didnt visit and my agency told me that I only need my 2006 tax return and W-2. Also my I-864 Affidavit of support and my last paystub. I have been seeing in some post something about having Aff. of Immunization. Is this for the child or my Immunizations. Can anyone give me some info on this.

The Affidavit of Immunization is the DS-230 form and relates to the immunization of the child. It essentially states that you will maintain the proper immunization for the child. Hope this helps.

Hi,
The affidavit for vacinations is a statement that is signed by you (and spouse, if married,), and notorized stating that you will get all needed vaccines for your child upon arrival in the U.S. or when age appropriate. It is basically for the embassy doc. If you don't need one Caleb may have had his shots or your agency requires him to receive any needed at the embassy appointment.

First of all, congratulations on your pick-up trip!!
Secondly, we just had our appt. last week and we also had not visited. We needed our passports, complete copy of last federal tax return (with all accompanying schedules and W-2's) with an attached notarized affidavit stating it was a true and accurate copy, the I-864, and $380 embassy fee. We had nothing at all concerning immunizations. I do know Camille's were current and up-to-date.
