Dossier documents - could someone PM me?
Hi Forum Friends,
Could someone please PM me the list of required documents for the dossier? I realize the list may vary by agency. Also, is a medical letter needed for children that states they are HIV negative? Thanks for your help. sb123 |

The medical forms seem to vary by agency, but at least for my agency, the medical form for children already in our family did NOT require a blood test confirming negative for HIV. I'll see if I can throw some things together reguarding the dossier and PM it to you.
In the meantime, just to give you an idea, our medical form for children already in our family asked the following: Part 1 Complete by Parents Yes/No Questions Has the child ever had treatment for a serious or chronic illness? Has this child been hospitalized in the past five years? Has this child ever received, or been advised to seek, mental health services? Has this child ever received or been advised treatment for alcohol/substance abuse? Has this child ever had a communicable disease? Does this child have a history of any hereditary disease? (If the answer to any of these questions is yes, please explain.) Part 2 - To be completed by Licensed Physician Weight, Height, Heart, Blood Pressure, Temperature, Abdomen, Pulse, Lungs, Nervous System Does this child suffer from an illness, including a communicable disease that would adversely affect the health and welfare of an adopted child placed in the home? Are there any chronic or serious disorders for which this child is receiving treatment? Is the Child currently taking medication? Is this child experiencing any physical, behavioural or emotional problems that would adversely affect an adopted child in this home? Have you referred this child to any other medical or mental health services? ***************************** So, it doesn't specifically require information reguarding HIV status of children already in the home, but the questions do seem to cover any serious/chronic illnesses. The medical form for my husband was nearly identical but went on to require additional blood tests specifically testing for HIV, Hep B, Hep C, Urinalysis, Tine or Mantoux, and a CBC. Hope this helps give you an idea of what our medical forms required (at least with our agency)

If you feel comfortable PMing me your email adress I can send you the document that lists everything we needed for our dossiers. I may still have our kids' medical form (not sure) and for my DH and myself we only needed a letter that we were in good health and able to parent - no form. No blood tests for any of us.
