What form DNA is everyone talking about?
I am reading that people are being denied DNA authorization because of some form that was signed and not typed. What form are we talking about? I am waiting for DNA authorization and now I am scared. Thanks.

It is a contact form that has two of your phone numbers, and an email address. This form had to be notarized and I think it went into effect sept 1.

I believe that this is the form that has your contact email on it. And the form itself, left the lines for the child's name blank. I wrote it all in, singed and got it notarized. But, my agency told me I had to redo it because they require it to be typed. So, I had to get the form again, type in the childs name and birthdate, etc and then notarize it again. Probably, some agencies just didn't catch that it had to be typed, so they didn't have their parents redo it and then it got denied.
