I've been reading on here about having to do witness statements. I wasn't told to do these. However, I have two "General Name Affidavit" forms to be filled out. Are these the same things as witness statements? I'm confused.

I really can't answer your question, but our witness statement form came from our agency with our dossier packet. Good luck!

The Name Affidavits are a letter stating all the names you are (and have been) known by. Make sure you double check, triple check, and then check it a fourth time against everything in your dossier to make sure every possible name, nickname, maiden name, married name, middle name possibility is listed.

The Witness Statements were form letters provided by our agency that we inserted the names of the person signing it, declaring that you are a good and moral person who can care for a child.

Then there are Letters of Reference which address your ability to parent a child and your financial and moral ability to care for a child. Like a letter of recommendation.

Keep in mind that dossier requirement can vary from one agency/attorney to the next, but we needed all three for our dossiers. And two of each.

we had to do witness statements, although we did them at the time of referral, along with the POA. the witness statements we had signed included the name of the child we were adopting.

i don't think they are necessary until the pgn stage, although i could be wrong and they use these for fc.

I needed Letters of Recommendation for my agency. They were not notarized, certified or authenticated.

I also received Witness statements to fill out from my agency. The witness had to fill in their name, address and how long they have known me. These had to be notarized, certified and authenticated. You should check with your agency since you wouldn't want to have anything missing from your file for PGN.

We also have to do both name affidavits and witness statements. Our witness statements are sent to us by our agency with all the info of our witness and then must be signed, notarized, certified, etc. I'd check!!
