New Form?

I was just informed of another form "they" need. Is this new or just another oversight?

Its from USCIS / top corner says american embassy guate. and it asks for email address and phone number. Needs to be notarized.

Anyone?

Starting in September the embassy will require two new forms when your attorney applies for DNA authorization. One is from the adoptive parents verifying their contact information (and it must be typed) and it has to be signed/notarized.

The other is from the agency verifying their contact information. Again it must be typed, signed, notarized.

Apparently the embassy has been catching flack about families not getting emails about pre-approval and they are saying that the information turned in was too difficult to read and therefore the emails were being put in incorrectly. These new typed forms are to help eliminate that problem.

But if DNA is requested/authorized before September, they don't need this new form?

I have the same question as Jenn... we're waiting on preapproval now.

Also, does this mean that the embassy notifies the family directly when they have preapproval? That wasn't the case with our first adoption, but it would be nice!

Guatadopt has the link to the new form. Apparently, it needs to be notarized but not authenticated. I have not seen any word yet on whether you have to complete this form if you already have DNA authorization but not results.

I see the form for the adoptive parents, but is there another separate form for the agency to use? Just wondering in case our agency isn't aware of this new form and don't have one...

Who sends the new form to the embassy? The attorney or the adoptive family? My attorney is working on getting authroziation this week - what if she doesn't have this new form? Do I mail it to the attorney or to the embassy?

I would guess that if you are already waiting for pre-approval, you don't need this form. Yes the Embassy will email you the pre-approval. I'm guessing that our agency gave the information to our attorney and he in turn gave it to the Embassy when he got our DNA auth. The Embassy is supposed to send the pre-approval email to you, your agency, and your attorney. Obviously they have been having problems with this, hence the new requirement.

I was confused because lionbird said she got a request from the embassy asking for this form and she started before us...I assumed we would be asked for it too...however, our attorney did ask our agency for my email address and he wasn't sure why...maybe it was for this reason...

I'm planning on doing it now just in case. We had a situation in spring where our FINAL PAPERS were submitted (for PINK, visa approval) to the embassy and we got a I-72 (error,missing paper) notification from the embassy via EMAIL -- so they do need your email just incase all the way to the end --
glad they had ours so we could follow up with the atty who had to run over and put in a new copy of a document needed. (he was sent an email also but didnt get it).

this document doesnt need much work -- just sigs and notary
