Hi,
I am getting my dossier ready. Does the medical note have to be notarized for physician signature or can i just sign in front of notary to verify it was physician that signed it? Hopefully this makes sense. My doctor's office doesn't have a notary in the clinic so I am unsure as to how to get it notaized with dr signature if it is her signature that needs to be notarized. Any suggestions? |

I think it varies by agency. Some agency's forms are set up in a way that requires the doctor's signature to be notarized. Our agency's form was specifically set upto not have the doctor's signature notarized. It had a statement under the doctor's signature where WE swore the information was true and then OUR signatures and sworn statement were notarized (we did have to go and appear before a notary the same day the doctor signed though, so the dates were the same on all our signatures and on the notary date).
You'll need to find a notary who will travel with you to the doctors office to get the doctor's signature notarized if your form is set up for the doctor's signature to be notarized. Hope this makes sense. Good luck!

look up on the internet, notary to go, they are wonderful. We had to use them for our MD signature. Not expensive at all, mine gave discount (only cost 20) b/c of adopting!

I have never heard of you having your signature notarized for the medical exam. We have done several dossiers, two for Guatemala, and we always needed to have the MD's signanture notarized but ask your agency. You can use a traveling notary or ask your MD to have it notarized while he/she is at his bank or another convenient place for him/her. We then re-emburse for notary cost. Anna

All three of our dossiers had to have medicals with a physicians signature notarized. We called a mobile notary and just had them meet us at the doctors office for his signature - oh by the way, we were charged for the office visit each time too - special, huh

I just had to have my signature notarized . . . not the doc's. It does vary by agency. We had no problems because of this.

We had to have the doctor's signature notarized twice (mistake the first time). The first time we looked up local notaries on the internet, and found one at our realtor's office. She volunteered to meet us there for free. The second time we used a traveling notary, again found on the internet (I googled "traveling notary"). She charged $5 per document, $10 for travel. Hopefully we won't have to go back a third time!
