General Medical Clearance Form
Hello All. I was wonderng if anyone knows where to find or has created a general medical clearance form that would be included in our dossier? I know it's nothing major, but I thought for sure I could find one online easily, but no luck. Thanks!!

we had to have one for the homestudy and one for the dossier. They were both very different. Best to wait as to what your SW gives you or your agency. My agency wouldn't accept the one that was for my homestudy. You don't want to have to do it again b/c that was the hardest for me to get and to get right.
