I decided to send in form G-884 and get the original documents for our children. I received a letter today stating that I need to send two copies of id for myself since I am requesting the information and proof of the relationship between myself and my children. What did you use to prove your relationship? adoption decree from Guatemala? baptism certificate? something else?

Can I ask where you got the form? Did you download it, or did you pick it up at some government office.
I'd like to fill one out and see what comes back. Thanks. Sorry I don't know what to send as Proof of relationship, or ID. I'm assuming a copy of your passport and the adoption decree would be good.

I think, I got the form sent to me. I don't think it can be downloaded. If you search this forum there was a thread a while back with lots of good information. It didn't address this question though.

I completed my re-adoption here before I submitted this form. From that process, I had a certificate of foreign birth issued here which listed me as her mom. So that's what I used.
You should also be listed on the new birth certificate from Guatemala as the mother. That might work if you have the certificate and an official translation. I guess the adoption decree would also work. I would probably lean towards more official paperwork (as opposed to baptismal records) if you have it available. I received a case number, but I am still waiting for documents / pictures from submitting that (about 4 months ago).

I have just started filling out this form and it can be downloaded from the internet. Legaleaze.com has a link to download it from their website. I have heard you can't get it from the USCIS website but can order it to be mailed to you.

For my ID, I sent copies of my drivers license and passport. They list what is acceptable on the front of the form. For the proof of relationship I used the adoption documents etc.
