Help with forms for the Embassy
Hi Everyone,
Plese help. Our program coordinator is out until the 23rd and we are leaving the 27th to pick up our daughter. I need help figuring out all these forms I have to take to the embassy My main concern is are we supposed to have the documents notarized here or are we to wait and sign them at the embassy there and have them notarized there. Also for the baby's legal name do we put her birth name or her birth name with our last name? Any help would be greatly appreciated. Thanks, Amy

We had to sign and notorized all our documents EXCEPT the I600. That one we only filled out Block A. Hope that is helpful. How do you add a new thread on this new forum? I am having trouble. Please pm me if possible.

Everything gets notarized in advance although I believe there is a notary at the Embassy. The child's name will be: First Name at birth, adoptive father's last name, adoptive mother's maiden name I do believe. Can someone else verify that? My wife does the hyphenated last name thing so I don't know if that had any impact on it.

I have a question. We did not visit our son. I am gathering the papers for the embassy and I have only one form the I-864 which states it needs to be notorized. I have copies of our last 3 years tax returns, current check stubs, copy of dossier, copy of BCIS approval letter, and the I-600 only partially filled out...the rest of the info I need to get form our attorney. Am I missing papers? Are there other forms which need notorized?
Any help would be greatly appreciated. Thanks! Kelly |

That's all that you need to have. Your attorney will help you finish the I-600 and you will need to sign it in front of the embassy official. Sounds like you're all set with the paperwork so relax and have an excellent pick up trip! Lori P.S. It is very common to feel like you're forgeting something right before the pick up!! ![]()

As far as what Kevin said about the baby's name... I would not count on it being the adoptive father's last name and then the adoptive mother's. Our baby's name was her given first and middle names, then our last name twice. My maiden name never came into it. Our agency said that it can happen either way and you don't know until you see it when you get there.

Our daughter's name was her first name, middle name, and my husband's last name, even though my last name is hyphenated. My maiden name did not show up.

Thanks everyone for the help! These forms are really a bear to figure out. Hopefully we are on the downward slope with all this paperwork!
