medical statement question
Ok. I am a little confused here. My homestudy agency has a medical statement that they use and I was wondering if I can use the same one for my Dossier? Does the one for the Dossier need to have my doc's letterhead? What all needs to be covered in the medical statement?

It all depends. Generally you need two different things for the homestudy and the dossier. In my case, the homestudy only had to get a form filled out, no notarizing necessary. For the dossier, we had to get a written letter, notarized, on the docs stationary..
