?? for anyone who has filled out the G-884

I was told by INS (or BCIS or whatever it is now) that I should fill this out in my name with my information as the legal parent of child whose records we are requesting. However, I was also told to fill in the alien registration number. That number is on my son's passport but if I'm filling out the form with my information, do I still need that number? Also, what did you put for specific information/documents desired? Thanks so much!

(I feel like such a dunce not being able to fill out this form!)

If this is an immigration form it can be ordered by phone at 1(800) 870-3676. I have not seen it in their list of forms either. Good Luck. Denise

I just did this last summer, and I saved a copy. Under "Name of Requester", I wrote my son's full name followed with "by" and then my full name, "mother." For example, "Isaac W. B. H. by Diana C. B. H., mother." I did put his alien registration number in that spot. Under "Specific Information", I wrote: "All information, documents, and photos in the file regarding the adoption and/or immigration of Isaac W.B.H. See attached letter." In the attached letter, I wrote down everything I had copies of already, along with any other items I thought might be in there such as photos of my son with his birth mother. On the Affidavit of Identity on the name line, I wrote "Diana C.B.H., and my son is Isaac W.B.H." and then for country of birth and birthdate I put my own on the lines and beneath the lines, I wrote "and that my son was born in Guatemala on March 7, 2002."

To obtain the forms, I just phoned BCIS at 1-800-870-3676 or 1-800-375-5283. I don't know why I have 2 different numbers. The latter number was given to me by my readoption attorney, though. While you're at it, ask for Form G-639 "Freedom of Information/Privacy Act Request." This one gets you copies of what is in your file, and you may be able to obtain some items this way that you can't with the G-884. I wasn't planning to use this one, but I've changed my mind after others recommended it.

I am having trouble getting documents back. My local office received my G-884 on June 30, and I still have not received any documents or even an acknowledgement that they're working on it. I've tried to call, but the 1-800 number is useless for status on G-884's, and without access to the local office's number, I'm stuck. I've asked my state's senator for help but haven't heard back from his office yet, either. I know some people have had success with this in less than 2 months, so I wish you luck for a speedy return.
