My fiancée and I will be married in May of this year, and I would like to adopt her 5-year old son. I have been his only father for the past 2 years and we have not heard from nor know the whereabouts of his biological father for that time, which seems to constitute "abandonment" as I have read regarding adoption consent.
However, I haven't been able to find any of the forms required if we were to submit this ourselves, rather than paying the costly fees associated with legal aid (we know about that from the expensive! cost of her divorce. If anyone can lead me to a site where I can download forms, or has any other suggestions / advice, it would be MUCH appreciated! Thanks- J |

forms for stepparent adoption
I don't know if your still looking but there really is no website to download forms. You have to file a petition at the with the prothonotary's office and they have sample petitions at the city/county building. Hope this helped.

Forms - without a lawyer
I am interested to see replies to this thread as I am trying to adopt my step daughter. Her biological father has agreed to consent, but I am unsure where to go now. In the next few days I will try the Court of Common Pleas. Any advice is appreciated.
