adult to be adopted

I'm 28 and my Dad (stepfather) wants to adopt me. He truely is my Dad and even though he has no biological children, he says he has 1 daughter. What steps do we need to take to make this adoption happen? If I take his last name will it become my maiden name? What are Mass. laws and regulations.
